Appointments
Upon scheduling your appointment, a major credit is required in order to confirm your visit, however, your card will not be charged unless you fail to show for your appointment (see cancellation policy below). Your insurance company will be notified of your visit and benefits, precertifications and authorizations will be obtained. If, however, you are uninsured or choose to pay for your visit, you are responsible for the appointment charges listed below.

The appointment charges are as follows:

New Patient/Follow-Up:	$250
Forensic:	$325

Payment Information
Payment must be made at the time of the office visit. We accept cash, personal check, MasterCard, Visa, American Express and private insurance. It is the patient’s responsibility to pay any deductible, coinsurance or any other amount not covered by an insurance company.

Cancellation Policy
If for any reason you are unable to keep your appointment, please cancel at least 24 business hours prior or missed appointment charges will be made.
